A Letter from Dr. Martin and Dr. Taylor,

Welcome to our practice! We are looking forward to meeting you and caring for your dental health. Our objectives are high quality, preventative dentistry, and your personal care. With this in mind, let us tell you what to expect on your first visit to our office.

A thorough examination will be completed, including necessary x-rays as well as other aids needed to make an accurate determination of the condition of your mouth, teeth and gums.

Fees and treatment plans will be given to you before treatment is begun. At that time we will go over estimated insurance out of pocket expenses if that is applicable. We would appreciate payment for the initial examination at this visit. Our front office coordinator will be happy to answer any questions you may have concerning financial arrangements. If you have dental insurance, please bring your dental insurance card with you. If no card was provided, please have insurance name, phone number, group number, and ID number. Our front office coordinator will be happy to assist you in filing for your reimbursement. As added convenience, we also accept Visa, MasterCard, Discover, American Express and CareCredit.

Enclosed you will find our health questionnaire. Please be as thorough as possible when you fill out the health history information sheet. It is vitally important for your protection that we know as much about your health as possible. We look forward to meeting you at your appointment.
